[Abstract]Objective We assessed the technical feasibility and efficacy of balloon or stent assisted detachable coiling
in the management of wide-necked intracranial aneurysms in humans.Methods 54 procedures of balloon or stent assisted detachable coiling were performed in 47 patients (22 women and 25 men) who had wide neck aneurysm. 31 patients had an acute subarachnoid hemorrhage. The mean diameter was 5.1 mm. Balloon-assisted Coil embolization was used in 14 procedures. We used Hyperform or Hyperglide non-detached balloon, which were placed in front of the ane urysm neck under road mapping. Then, the microcatheter for coil deployment was placed in the sac of the aneurysm. The coil was deployed in the aneurysm while the balloon was inflated temporarily, occluding the aneurysmal neck and parent artery. This process was repeated until the aneurysm had been embolized as well as possible. Stent assisted coiling was used in 40 procedures. The stent was deployed to cover the neck of the aneurys. After deployment of the stent, detachable coils were introduced into the aneurysm and detached as usual until occlusion is achieved. Results All procedures succeeded. Total occlusion was achieved in 46 of 54 aneurysms. Subtotal occlusion was achieved in 8 aneurysms. Incomplete occlusion was achieved in 8 aneurysms. Thromboembolic events occurred in 1 patient who was completed recovered 1 month later. No re-bleeding occurred. Conclusion Balloon-assisted Coil embolization is a safe and effective solution to wide-necked intracranial aneurysms.
